Delegate Lewis moves to amend Com Sub for HB4196 on page 4, Section 3, line 92 after the semicolon by striking out the word "and";

And,

On page 4, Section 3, line 95 by striking out the period, inserting in lieu thereof a semicolon and the following:

"and

(3) The patient shall acknowledge, in writing, receipt of the contraceptive counseling."

And,

On page 11, Section 5, line 175, after the semicolon by striking out the word "and" ;

And,

On page 11, Section 5, line 178, by striking out the period, inserting a semicolon and the following

"and

(iii) The patient shall acknowledge, in writing, receipt of the contraceptive counseling."
